Alzheimer's disease is the most common form of dementia, affecting up to 80 percent of all individuals with any form of dementia in the United States. An estimated 5.8 million people in the United States had Alzheimer's disease in 2020, and this number is projected to grow considerably with the aging of the large group of the Baby Boomers, born in the years 1946-1964. According to the Alzheimer's Association, by 2025, there will be 7.1 million Americans with Alzheimer's, a 22 percent increase from 2020. After diagnosis with Alzheimer's disease, the average person lives up to 8 more years, although some die sooner or much later. Non-Alzheimer's dementia is also a huge and growing problem in the United States and the world. In 2020, the Alzheimer’s Association estimated there were millions suffering from some other form of a degenerative brain disease that cannot be cured. Such other forms of dementia include vascular dementia, frontotemporal lobe dementia, dementia with Lewy bodies, and Parkinson's disease dementia. Less common forms of dementia include the dementia that is associated with Huntington's disease and Creutzfeldt-Jakob disease. In view of the growing prevalence of AD worldwide, there is an urgent need for the development of better diagnostic tools and more effective therapeutic interventions. Indeed, much work in this field has been done during last decades. As such, a major goal of current clinical research in AD is to improve early detection of disease and presymptomatic detection of neuronal dysfunction, concurrently with the development of better tools to assess disease progression in this group of disorders. All these putative correlates are commonly referred to as AD-related biomarkers. The ideal biomarker should be easy to quantify and measure, reproducible, not subject to wide variation in the general population and unaffected by co- morbid factors. For evaluation of therapies, a biomarker needs to change linearly with disease progression and closely correlate with established clinico-pathological parameters of the disease. There is growing evidence that the use of biomarkers will increase our ability to better indentify the underlying biology of AD, especially in its early stages. These biomarkers will improve the detection of the patients suitable for research studies and drug trials, and they will contribute to a better management of the disease in the clinical practice. Indeed, much work in this field has been done during last decades. The vast number of important applications, combined with the untamed diversity of already identified biomarkers, show that there is a pressing need to structure the research made on AD biomarkers into a solid, comprehensive and easy to use tool to de deployed in clinical settings. To date there are few publications compiling results on this topic. That is why when I was asked to address this task I accepted inmediately. Given the success of Volume I of this Research Topic, we are pleased to announce the launch of Volume II: “The Alzheimer's Disease Challenge”. The repeated failure of clinical trials on the amyloid-based medications and the pessimistic calculations of Alzheimer's disease cost burden for the next few decades present a severe challenge to humankind with severe social implications. In recent years, several alternative diagnostic and treatment procedures have been presented to treat and manage Alzheimer’s disease as it has been nearly impossible to suggest a holistic solution. Several revelations in human biology have highlighted the multiparametric character of the disease. Besides the amyloid aggregation and neurofibrillary tangles that result in Aβ toxicity and tau phosphorylation, processes such as Gene Mutations, Proteins Misfolding, Brain Biochemical and Histopathological Changes, Behavioral Changes, Nutrition and Metabolism Alterations, and Autonomic Dysfunctions due to Central Nervous System dysregulations are common signs and probably early diagnostic biomarkers in most of the Alzheimer's classification categories.

This publication is a landmark work commemorating the centennial of Alois Alzheimer's discovery of what would be known as Alzheimer's disease (AD). The centennial of Alois Alzheimer’s original description of the disease that would come to bear his name offers a vantage point from which to commemorate the seminal discoveries in the field. It traces how the true importance of AD as the major cause of late life dementia ultimately came to light and narrates the evolution of the concepts related to AD throughout the years and its recognition as a major public health problem, with an estimated 30-40 million people affected by AD today. To identify the breakthroughs, the editors have used citation analysis, landmark papers identified by current researchers, and drew upon their own experience and insights. This process took into account the perspectives of individuals who recall the impact of findings at the time they were made, as well as of scientists today who have the advantage of hindsight in weighing the lasting influence of these findings. Because modern AD research was triggered by the seminal work of Tomlinson, Blessed, and Roth some four decades ago, it is particularly fortunate that the vast majority of these milestone authors are still with us.
