This monograph is intended to compile the lectures presented at the 4th Annual Symposium "Cardiac Surgery:1992" held at the Frenchman's Reef Beach Resort, St. Thomas, US Virgin Islands, November 7-10, 1991. This symposium was organized by the Division of Cardiothoracic Surgery and the School of Cardiovascular Perfusion, Cooper Hospital/University Medical Center, Camden, New Jersey and sponsored by the University of Medicine and Dentistry of New Jersey, Robert Wood Johnson Medical School at Camden, Philadelphia Academy of Cardiovascular Perfusion and the American Society of Extracorporeal Technology -Region II. The symposium was devoted to cardiothoracic surgeons, cardiologists, cardiovascular perfusionists, and nurses involved in the management of the cardiac surgical patient. The beginning of cardiac surgery is generally considered to be the successful repair of a wound of the heart that took place on the 7th of September 1896 in Frankfurt am Main. This operation put an end to the widespread belief that nature had set the heart beyond the limits of surgery. The successive development of heart surgery moved together with other advances that were rapidly taking place in various fields of medicine and surgery and which, already in the first half of the 20th century, had allowed surgical pioneers to successfully correct a number of congenital and acquired heart diseases by adopting closed-heart techniques. Undoubtedly, the most notable progress in the history of cardiac surgery took place between the second half of the 1950s and the end of the 1960s with the introduction of extra-corporeal circulation that allowed surgeons to perform interventions under direct vision within the bloodless heart chambers. This fundamental technological innovation fostered the development of surgical procedures that are still adopted to this day. Among these that must be mentioned are the correction of complex congenital heart diseases, the designing and creation of implantable prosthetic heart valves, the introduction of coronary artery surgery, the repair of severe diseases of the aorta, the commencement of heart transplantation, and the first implantation of an artificial heart. This book narrates these fascinating and sometimes dramatic events, as well as detailing some of the greatest pioneers of cardiac surgery.

Cardiac surgery has developed dramatically since the first open-he art operations were performed in the mid 1950s. Although the improvement of surgical technique, extracorporeal circulation, and postoperative management has contributed to a marked reduction of morbidity and mortality, the development of cardiac surgery to its present state would not have been possible without blood substitution by homologous donor blood. Only 20 years ago, open-heart operations required an average of 8 units of blood preserves. The excessive need of donor blood in those early days was mainly due to premature surgical technique, insecure control of anticoagulation, severe blood trauma by extracorporeal circulation, and the lack of retransfusion technologies that would have allowed the reuse of shed mediastinal blood. The introduction of new technologies, such as normovolemic hemodilution, in traoperative autotransfusion, postoperative return of shed mediastinal blood, and predonation of autologous blood has greatly reduced donor blood requirements. At present the majority of routine coronary artery surgical procedures can be performed without any blood transfusion. Blood loss, however, may be considerable in patients undergoing complex valve surgery or reoperations, as they often require several units of transfused blood. Blood conservation has now become an area of major interest for the cardiac surgeon. This increased concern is caused by infectious complications of blood transfusion, in particular hepatitis and, more recently, AIDS.

In 1962, Thomas Kuhne coined the term “paradigm shift” while arguing that human knowledge advances by quantum leaps with interspersed smaller steps. Preparation for the major advance is generally not a concerted effort by thought leaders. Rather, a few (or one) visionaries gain insights into a process and are able to definitely demonstrate the accuracy of their worldview. Often, the epiphany does not occur during the intellectual lifetime of the discoverers. Medicine has had numerous such “paradigm shifts” including the compelling reworking of Galen’s concepts of the body. Of note, the scientific world of the time explained the new views by arguing that the human body must have changed between the time of ancient Greece and modern Europe. The inauguration of cardiac surgery itself required profound shifts in medicine’s view of physiology. Yet, over the ensuing 40 years, the field was fine tuned so we could provide greater than 95% success rates in elective surgery with low cost and short h- pital stays. In some parts of the world, the procedures were viewed as commodities and prices dropped as providers were unable to differentiate the quality of their work. As patients and their physicians became more demanding, the desire to make the procedures “minimally invasive” grew. In effect, what we were really searching for was a life saving procedure that also preserved quality of life. In short, “minimally invasive” has really been a code phrase for procedures that disrupt our quality of life the least.
