From February 24 -28, 1992 an international symposium on Durability of Disease Resistance was held at the International Agricultural Centre in Wageningen, the Netherlands. The symposium, organized by the Department of Plant Breeding of Wageningen Agricultural University and the Centre for Plant Breeding and Repro duction Research, CPRO-DLO, was part of the DGIS funded programme Durable Resistance in Developing Countries. Without any form of prevention or protection nearly all crops will be seriously or even severely damaged by a range of pathogens. In modern agriculture man has been able to control many if not most pathogens using i) pesticides, ii) phyto sanitary methods such as control of seed and plant material in order to start a crop disease free, iii) agronomic measures such as crop rotation, iv) disease resis tance or combinations of these measures. Over the years the use of pesticides has increased enormously and so did the pro blems associated with pesticide use, such as environmental pollution and building of resistance and tolerance to these pesticides in the pathogens. The use of resis tance too increased strongly over the years and here too problems arose.

RESISTANCE AND THE PROCESS OF THERAPEUTIC CHANGE Paul L. Wachtel Psychotherapy, whether practiced from a psychodynamic or a behavioral point of view,! is rarely as straightforward as textbooks and case reports usually seem to imply. More often the work proceeds in fits and starts (and often does not seem to be proceeding at all, but rather unraveling or moving backward). The "typical" case is in fact quite atypical. Almost all cases present substantial difficulties for which the therapist feels, at least some of the time, quite unprepared. Practicing psychotherapy is a difficult-if also rewarding-way to earn a living. It is no profession for the individual who likes certainty, predictability, or a fairly constant sense that one knows what one is doing. There are few professions in which feeling stupid or stymied is as likely to be a part of one's ordinary professional day, even for those at the pinnacle of the field. Indeed, I would be loath to refer a patient to any therapist who declared that he almost always felt effective and clear about what was going on. Such a feeling can be maintained, I believe, only by an inordinate amount of bravado and lack of critical self-reflection. This book describes antibiotic resistance amongst pathogenic bacteria. It starts with an overview of the erosion of the efficacy of antibiotics by resistance and the decrease in the rate of replacement of redundant compounds. The origins of antibiotic resistance are then described. It is proposed that there is a large bacterial resistome which is a collection of all resistance genes and their precursors in both pathogenic and non-pathogenic bacteria. Ongoing resistance surveillance programs are also discussed, together with the perspective of a clinical microbiologist. The book then turns to specific themes such as the most serious area of resistance in pathogens, namely in Gram-negative organisms. The role of combinations of antibiotics in combating resistance emergence is discussed, particularly in the tuberculosis field, and then the importance of non-multiplying and persistent bacteria which are phenotypically resistant to antibiotics and prolong the duration of therapy of antibiotics which leads to poor compliance and resistance emergence. The role of anti-microbial compounds in textiles is covered, with its potential to exacerbate the spread of resistance. Then, efflux pumps are discussed. The final chapter describes the compounds which are in late stage clinical development, illustrating the paucity of the antibiotic pipeline, especially for Gram-negative bacteria.
