BOOK EXCERPT:
As life expectancy is extended, the incidence and occurrence of Aortic Aneurysms are becoming increasingly common. Until recently, the treatment of aortic disease was primarily surgical, carrying a high mortality rate among the high risk elderly population. In Aortic Aneurysms: Pathogenesis and Treatment, Gilbert R. Upchurch Jr., Enrique Criado, and a panel of distinguished physicians comprehensively assess the rapidly evolving field of aneurysm treatment, focusing on aneurysms of the descending thoracic and abdominal aorta. The discussion of Aortic Aneurysms covers endovascular aortic repair as well as the recent significant improvements in stent graft design. This volume provides a concise and authoritative view of the current state of patient management, serving as a primer on clinical care and surgical procedures for aortic aneurysms. Comprehensive and up-to-date, Aortic Aneurysms: Pathogenesis and Treatment, offers Cardiologists, Vascular Surgeons, and Thoracic and Cardiac Surgeons a multidisciplinary approach to the management and treatment of patients with aortic disease.

BOOK EXCERPT:
Aortic aneurysms are very silent, insidious, and sudden complicated pathologies with high incidence of sudden death. The aorta has several different parts, which are affected by aneurysmal pathology independently and separately or conjointly. Preventive medicine and determinative genetic heritage are the first steps during diagnosis and precaution. Preventive screening of the general population seems the best way for diagnosing early asymptomatic aneurysms with increased health cost. Because sudden death is the first symptom of aortic aneurysms, early interventional treatment is essential to protect fatal complications. In spite of open surgical repair which is the standard procedure, isolated or hybrid endovascular approaches are used widespread in the last decade due to reduction of procedural complications. This book is composed of the main topics on aortic aneurysms and surgical treatments to extend scientific and therapeutic perspective and vision of cardiac teams.

BOOK EXCERPT:
This book considers mainly etiology, pathogenesis, and pathophysiology of aortic aneurysms (AA) and aneurysm rupture and addresses anyone engaged in treatment and prevention of AA. Multiple factors are implicated in AA pathogenesis, and are outlined here in detail by a team of specialist researchers. Initial pathological events in AA involve recruitment and infiltration of leukocytes into the aortic adventitia and media, which are associated with the production of inflammatory cytokines, chemokine, and reactive oxygen species. AA development is characterized by elastin fragmentation. As the aorta dilates due to loss of elastin and attenuation of the media, the arterial wall thickens as a result of remodeling. Collagen synthesis increases during the early stages of aneurysm formation, suggesting a repair process, but resulting in a less distensible vessel. Proteases identified in excess in AA and other aortic diseases include matrix metalloproteinases (MMPs), cathepsins, chymase and others. The elucidation of these issues will identify new targets for prophylactic and therapeutic intervention.

BOOK EXCERPT:
This treatise offers an extensive exploration of aortic aneurysm, a critical cardiovascular condition with profound implications. Spanning numerous chapters, each meticulously crafted, the treatise provides an in-depth journey into the realms of definition, classification, epidemiology, etiology, and risk factors. Readers are guided through the intricate landscape of aortic anatomy, layers, and segments, unveiling the complexities of both thoracic and abdominal aorta. Genetic factors and extracellular matrix components are dissected, unraveling the intricate web of molecular underpinnings. Delving into the pathophysiology, the treatise unveils the inflammatory pathways and enzymatic cascades contributing to aneurysm formation. Each chapter unfolds a layer of knowledge, elucidating the signs, symptoms, and diagnostic imaging techniques employed in clinical settings. Size-based and etiology-based classifications provide a structured framework for understanding the diverse presentations of aortic aneurysms, further complemented by a meticulous grading of severity and risk assessment.

BOOK EXCERPT:
This book considers mainly diagnosis, screening, surveillance and treatment of abdominal, thoracoabdominal and thoracic aortic aneurysms. It addresses vascular and cardiothoracic surgeons and interventional radiologists, but also anyone engaged in vascular medicine. The high mortality of ruptured aneurysms certainly favors the recommendation of prophylactic repair of asymptomatic aortic aneurysms (AA) and therewith a generous screening. However, the comorbidities of these patients and their age have to be kept in mind if the efficacy and cost effectiveness of screening and prophylactic surgery should not be overestimated. The treatment recommendations which will be outlined here, have to regard on the one hand the natural course of the disease, the risk of rupture, and the life expectancy of the patient, and on the other hand the morbidity and mortality of the prophylactic surgical intervention. The book describes perioperative mortality after endovascular and open repair of AA, long-term outcome after repair, and the cost-effectiveness of treatment.
