Coronary Graft Failure

eBook Download

BOOK EXCERPT:

Coronary artery bypass surgery has been developed since 1960s to overcome proximal coronary artery disease. Worldwide, the number of patients that are undergoing coronary artery bypass surgery is steadily increasing. Depending on diverse risk factors, one fifth of grafts are occluded at 1 year. For the remaining, graft patency last usually 8–15 years. This book brings together the main specialists in the field to review the current evidence on epidemiology, pathophysiology, diagnostic, new imaging techniques and specific therapeutic modalities. This volume aims to update a complex subject represented by coronary graft failure. The authors of this monograph are interventional cardiologists, cardiovascular surgeons and research scientists, who will be creating four parts and 71 chapters that are divided in order to give a uniform interpretation of this condition including all aspects of coronary graft failure This book not only provides the most up-to-dated scientific evidence in the field but in a two-step manner. Each chapter is divided into a at a glance part that reflects the basic evidence on the topic, and a “full picture” part that brings all what the advanced reader should be brought with.

This book is a comprehensive overview of heart failure and cardiac transplantation and integrates scientific and clinical information about the physiology, pathophysiology, diagnosis, and treatment of this disorder. Organized into five parts, it reviews the history and basic mechanisms of heart failure; etiology of heart failure; heart failure disease progression; advanced therapies for heart failure; and cardiac transplantation. The book presents basic concepts in the physiology, molecular biology, pathology, and epidemiology of the normal and failing heart; known causes of heart failure, such as right heart failure, valvular cardiomyopathy, molecular mechanisms of sarcomeric cardiomyopathies, and neuromuscular cardiomyopathy; cardiorenal syndrome; neurohormonal activation; cardiac resynchronization, ventricular assist devices; regenerative mechanisms; orthotopic heart transplantation; early and late management of the post-transplant patient; heart transplantation and antibody-mediated rejections; heart-lung transplantation; and cardiac xenotransplantation. Featuring contributions from leaders in the fields of heart failure, cardiac transplantation, cardiac pathology, and cardiovascular molecular research, Congestive Heart Failure and Cardiac Transplantation is a valuable compendium for cardiologists, cardiothoracic surgeons, researchers, trainees, and students.

Off Pump Coronary Artery Bypass

eBook Download

BOOK EXCERPT:

This book is a valuable resource on the subject of off-pump coronary artery bypass (OPCAB) surgery, which has recently been developed as a promising technique for reducing surgical damage to patients. Because of the increasing number of older patients who are candidates for a coronary artery bypass graft (CABG) operation, less-invasive surgeries are becoming a priority all over the world. Despite some challenges, such as its technical difficulties, emerging questions, and controversies involved, OPCAB surgery in CABG is now the gold standard in Japan. This book, written by OPCAB experts, examines the procedure established and its chief benefits. Its coverage ranges from a careful evaluation of the evidence for and against OPCAB surgery to preoperative and intraoperative management of patients undergoing coronary bypass surgery. It also contains a description of advanced techniques for every aspect of the procedure. The authors[share essential insights into the latest trends in cardiovascular surgery for professionals and residents, presenting detailed descriptions of the exposure of the heart, stabilization of each coronary artery target, construction of distal anastomoses, and other important factors. The book offers an invaluable reference guide for cardiac surgeons, cardiologists, anesthesiologists, and other medical staff in operating rooms around the world.

BOOK EXCERPT:

Cardiac transplantation has had a major impact on the quality of life and longevity of an ever-increasing number of patients. This benefit is significantly eroded by the development of an accelerated form of coronary arterial disease which shows some, but not all, of the characteristics of native coronary artery disease, and itself is one of the ma

BOOK EXCERPT:

This book addresses one of the largest unmet needs in transplantation, the need to reduce late allograft loss. In the current era, it is reasonable to expect that most allografts will serve their recipients through their life span and death with preserved graft function the ultimate goal for all transplant recipients. However, long term allograft s

Minimally Invasive Cardiac Surgery

eBook Download

BOOK EXCERPT:

In 1962, Thomas Kuhne coined the term “paradigm shift” while arguing that human knowledge advances by quantum leaps with interspersed smaller steps. Preparation for the major advance is generally not a concerted effort by thought leaders. Rather, a few (or one) visionaries gain insights into a process and are able to definitely demonstrate the accuracy of their worldview. Often, the epiphany does not occur during the intellectual lifetime of the discoverers. Medicine has had numerous such “paradigm shifts” including the compelling reworking of Galen’s concepts of the body. Of note, the scientific world of the time explained the new views by arguing that the human body must have changed between the time of ancient Greece and modern Europe. The inauguration of cardiac surgery itself required profound shifts in medicine’s view of physiology. Yet, over the ensuing 40 years, the field was fine tuned so we could provide greater than 95% success rates in elective surgery with low cost and short h- pital stays. In some parts of the world, the procedures were viewed as commodities and prices dropped as providers were unable to differentiate the quality of their work. As patients and their physicians became more demanding, the desire to make the procedures “minimally invasive” grew. In effect, what we were really searching for was a life saving procedure that also preserved quality of life. In short, “minimally invasive” has really been a code phrase for procedures that disrupt our quality of life the least.
