This report describes the current situation with regard to universal health coverage and global quality of care, and outlines the steps governments, health services and their workers, together with citizens and patients need to urgently take.

In 2015, building on the advances of the Millennium Development Goals, the United Nations adopted Sustainable Development Goals that include an explicit commitment to achieve universal health coverage by 2030. However, enormous gaps remain between what is achievable in human health and where global health stands today, and progress has been both incomplete and unevenly distributed. In order to meet this goal, a deliberate and comprehensive effort is needed to improve the quality of health care services globally. Crossing the Global Quality Chasm: Improving Health Care Worldwide focuses on one particular shortfall in health care affecting global populations: defects in the quality of care. This study reviews the available evidence on the quality of care worldwide and makes recommendations to improve health care quality globally while expanding access to preventive and therapeutic services, with a focus in low-resource areas. Crossing the Global Quality Chasm emphasizes the organization and delivery of safe and effective care at the patient/provider interface. This study explores issues of access to services and commodities, effectiveness, safety, efficiency, and equity. Focusing on front line service delivery that can directly impact health outcomes for individuals and populations, this book will be an essential guide for key stakeholders, governments, donors, health systems, and others involved in health care.

Quality health services and palliative care are inextricably linked and both vital for achievement of universal health coverage (UHC). As countries commit to achieving UHC, it is imperative to ensure that the design and delivery of palliative care services place attention on quality of care, with action needed across all domains of quality health services: effectiveness; safety; people-centredness; timeliness; equity; integration; and efficiency. Providing compassionate, dignified and people-centred palliative care is an ethical responsibility of health systems. Progress towards UHC is at risk unless urgent action is taken to increase the provision of quality palliative care to relieve serious health-related suffering and guarantee dignity during the end of life for all people. This document provides a practical resource to support implementation of sustainable improvements in the quality of palliative care. It describes approaches to quality policy, strategy and planning for palliative care programmes and services, presents learning on quality of care arising from palliative care programmes, and offers considerations on measurement of quality palliative care services at all levels of the health system. The document also highlights relevant World Health Organization resources available that further support the development of quality palliative care services. The audience for this document is a general one that includes policy-makers, palliative care service planners, managers, practitioners and health care providers at all levels.

This book examines the idea of a fundamental entitlement to health and healthcare from a human rights perspective. The volume is based on a particular conceptual reasoning that balances critical thinking and pragmatism in the context of a universal right to health. Thus, the primary focus of the book is the relationship or contrast between rights-based discourse/jurisprudential arguments and real-life healthcare contexts. The work sets out the constraints that are imposed on a universal right to health by practical realities such as economic hardship in countries, lack of appropriate governance, and lack of support for the implementation of this right through appropriate resource allocation. It queries the degree to which the existence of this legally enshrined right and its application in instruments such as the International Covenant on Economic, Social and Cultural Rights (ICESCR) and the Universal Declaration of Human Rights (UDHR) can be more than an ephemeral aspiration but can, actually, sustain, promote, and instil good practice. It further asks if social reality and the inequalities that present themselves therein impede the implementation of laudable human rights, particularly within marginalised communities and cadres of people. It deliberates on what states and global bodies do, or could do, in practical terms to ensure that such rights are moved beyond the aspirational and become attainable and implementable. Divided into three parts, the first analyses the notion of a universal inalienable right to health(care) from jurisprudential, anthropological, legal, and ethical perspectives. The second part considers the translation of international human rights norms into specific jurisdictional healthcare contexts. With a global perspective it includes countries with very different legal, economic, and social contexts. Finally, the third part summarises the lessons learnt and provides a pathway for future action. The book will be an invaluable resource for students, academics, and policymakers working in the areas of health law and policy, and international human rights law.

Healthcare quality (HQ) became eminent during the COVID-19 crisis. Despite the efforts made by the Ministry of Health in Oman, the country faced several challenges, such as a lack of available funds and patients perceived in a negative manner which hindered their treatment experience. The study examines factors affecting HQ using the SERVQUAL model; patient satisfaction in Omani public hospitals, including healthcare quality, resources availability (RA), and healthcare facility preparedness (HFP); and the moderation effect of the perceived risk of COVID-19 pandemic. This research uses a mixed method with a sample size of 387 patients (quantitatively) selected using probability random sampling of 8 patients (qualitatively) who visited public hospitals The findings from the quantitative analysis reveals that the tangibility and RA had insignificant effect on the HQ. The moderation effect of perceived risk of COVID-19 between HQ, RA, HFP and patient satisfaction was insignificant. The results of the qualitative analysis indicated other factors that affected patient satisfaction, including communication, helpfulness, compassion, physician professionalism, patient-doctor contact, waiting time, atmosphere, and cleanliness. The study proposed a strategy for technical categories that include professional skills, service outcomes and concrete quality aspects, and functional categories. The study has also recommended implications for further investigation.
