The purpose of this workbook is to assist ministries of health, health managers and practitioners in engaging with the private sector on delivery of quality maternal, newborn and child health (MNCH) services in lower- and middle-income countries. Private health care is one of the fastest growing segments of the health-care system in lower- and middle-income countries, and private providers are an important source of health care. To accelerate progress to reach the Sustainable Development Goals for ending preventable maternal, newborn and child deaths, it is critical that whole health system organizations invest not only in increasing coverage of interventions, but also in quality. The audience for the workbook is those who are involved with organizing and implementing processes for engaging the private sector in delivery of quality MNCH services.

This case study documents the experience of engaging with the private sector in health to maintain the delivery and use of essential health services (EHS) with a specific focus on maternal, newborn and child health (MNCH) and family planning (FP) services during the COVID-19 pandemic in Pakistan. A case study methodology was employed, drawing on desk review and key informant interviews, which were conducted between February and April 2022. Several opportunities were raised through the case study, to seize momentum, to ‘build back’ and nurture trust in the health system eroded by COVID-19 pandemic, harnessing all health sectors. While these were specific to Pakistan, they can be relevant to a wider audience.

Poor access to care in low- and middle-income countries due to high costs, geographic barriers, and a shortage of trained medical staff has motivated many organizations to rethink their model of health service delivery. Many of these new models are being developed by private sector actors, including non-profits, such as non-governmental organizations, and for-profits, such as social enterprises. By partnering extensively with public sector organizations, these non-state actors have enormous potential to scale innovation in global health. Since the private health sector is an important, and often dominant, provider of health services in Sub-Saharan Africa, it is the job of governments as the stewards of the health system to engage with it. Increasing the contributions that the existing private health sector is making to public health is an important, but often neglected, element of meeting the daunting health-related challenges facing African nations. This Report presents newly collected data on how and how effectively each country in the Africa region is engaging the respective private health sectors; and how the engagement compares across the region. While the approach taken by governments varies greatly between countries, there is much room for improvement in the Africa region overall to engage more effectively and room for exchange of ideas and good practices on how to do so. Improved solutions on the policy/regulatory side should be supported by effective organization of the private sector itself and by adjustments in donor programs that take the dynamics of the private health sector better into account.

Millennium Development Goal 5 is to improve maternal health, and its bold target is to reduce the maternal mortality ratio by three quarters between 1990 and 2015. Still, each year more than half a million women die from pregnancy-related causes that are avoidable. At the present rate of progress, the world will fall well short of the MDG 5 target. This report details progress in maternal health and highlights areas where improvements are needed.
