Hypertension is a major world-wide health problem. With high blood pressure there is a greater risk of stroke, heart attack, heart failure, kidney disease and renal failure. Far too few people realize what the risks are and what can be done to prevent these risks even in the countries where pro grams in hypertension research are active and the full significance of hyper tension is best understood. Some studies of the known hypertensive popu lation indicate that one-half or less are receiving adequate treatment, and, of those on therapy, only half have their high blood pressure satisfactorily controlled. These realizations emphasize the need to inform all segments of society throughout the world on the importance of detection and control of high blood pressure. The great incidence of hypertension makes it of para mount importance that all practicing physicians have available the latest information on diagnosis and treatment of hypertensive cardiovascular dis ease. This treatise on hypertension arrives at a time when there is an increasing recognition the world over of the importance of detecting and treating high blood pressure. The book has been edited by Dr. A. Amery and his asso ciates in the University of Leuven. Professor Amery is one of the leaders in the field of hypertension and serves on the Council of the International Society of Hypertension.

It is not often that one one writes the foreword for a book based on a conference which contributed so much to our knowledge in the field of hypertension. During my close association with the International Society of Hypertension from 1978-1982, numerous satellite symposia were held in connection with five interna tional meetings. The specific topics addressed in this volume, the quality of the contributions, and the superb expertise of the contributors make this monograph one of the most outstanding publications that the International Society of Hyper tension has sponsored. Satellite symposia such as the present one serve useful and important functions for our society. They provide a mechanism of getting together the outstanding experts on special subjects for presentation of new data and for a free interchange of ideas. This type of endeavor is one of the most uniquely effective ways of accumulating new knowledge, because the data presented are subjected to critical review and discussion. No textbook or publication in journals can provide the type of critically evaluated information that comes from a small group is a mechanism for scientists to become meeting of this type. Furthermore, it acquainted or better acquainted with one another. All of this is, of course, very provocative and supportive of high quality research, one of the trademarks of the International Society of Hypertension. The Stressed Heart is truly unique in concept and will provide an eXCItmg adventure to the reader no matter what his or her field of expertise and interest. The title, although quite appropriate, does not adequately indicate the range of topics considered or the rational interrelationships among them. Indeed, perhaps the most important point to be learned from the book is that a serious consideration of the response of the heart to mechanical overload, ischemia, or excessive humoral stimuli must include evaluation of each of the topics in the table of contents. The heart responds to stress through alterations in both structure and function. How these changes are brought about is the subject of the initial chapters. These consider first the normal regulation of gene expression in the heart, the rapid response to mechanical overload that leads to both quantitative and qualitative changes in the contractile proteins, and our current understand ing of the signals that might be elicited by stress and alter gene expression. One chapter emphasizes the fact that, regardless of the nature of the stress, the common denominator is a discrepancy between energy requirements and expenditure. The central role of cellular acidosis in initiating the sequence of responses to stress and the possible roles of peptide regulators of transcription and protein regulators of translation are considered in detail.

In Groningen there has been a continuous investigation of hypertrophic cardio myopathy for almost twenty years. Members of this working-group have tried to study the many aspects of this intriguing disease, and it is in this volume that they present their recent results and current concepts (chapters 2, 3, 5 and 6). A contribution from the Dutch interuniversity Institute of Cardiology presenting a detailed account of evaluation by means of radionuclide techniques completes the non-invasive picture of cardiomyopathies (chapter 4). On July 6th, 1984, a symposium entitled 'Recent views on hypertrophic cardio myopathy' was held at the University Hospital Groningen. The main reason to organize this meeting was the retirement of one of our working-group members, namely E. van der Wall, as head of the cardiological department. The speakers at the symposium presented their lectures based on their contributions to this book. It is an honour for us to have obtained their full cooperation for this book and for the symposium. We are very grateful that this Groningen manifestation could include the outstanding contributions of several well-known experts from abroad. We there fore thank our colleagues Profs. Epstein, Goodwin, Kaltenbach and Schulte for their spontaneous and valuable cooperation (chapters 1, 7,8 and 9).
