Integrating Social Care Into The Delivery Of Health Care

eBook Download

BOOK EXCERPT:

Integrating Social Care into the Delivery of Health Care: Moving Upstream to Improve the Nation's Health was released in September 2019, before the World Health Organization declared COVID-19 a global pandemic in March 2020. Improving social conditions remains critical to improving health outcomes, and integrating social care into health care delivery is more relevant than ever in the context of the pandemic and increased strains placed on the U.S. health care system. The report and its related products ultimately aim to help improve health and health equity, during COVID-19 and beyond. The consistent and compelling evidence on how social determinants shape health has led to a growing recognition throughout the health care sector that improving health and health equity is likely to depend â€" at least in part â€" on mitigating adverse social determinants. This recognition has been bolstered by a shift in the health care sector towards value-based payment, which incentivizes improved health outcomes for persons and populations rather than service delivery alone. The combined result of these changes has been a growing emphasis on health care systems addressing patients' social risk factors and social needs with the aim of improving health outcomes. This may involve health care systems linking individual patients with government and community social services, but important questions need to be answered about when and how health care systems should integrate social care into their practices and what kinds of infrastructure are required to facilitate such activities. Integrating Social Care into the Delivery of Health Care: Moving Upstream to Improve the Nation's Health examines the potential for integrating services addressing social needs and the social determinants of health into the delivery of health care to achieve better health outcomes. This report assesses approaches to social care integration currently being taken by health care providers and systems, and new or emerging approaches and opportunities; current roles in such integration by different disciplines and organizations, and new or emerging roles and types of providers; and current and emerging efforts to design health care systems to improve the nation's health and reduce health inequities.

Integrating The Organization Of Health Services Worker Wellbeing And Quality Of Care

eBook Download

BOOK EXCERPT:

This book introduces the concept of ‘healthy healthcare’ and posits that this new concept is necessary in light of a shortage of healthcare staff in the near future. Healthy healthcare implies that healthcare systems are designed, managed and financed in balance with the available resources to improve workers’ health and performance. Ultimately, a balanced perspective taking into account the patient, the staff and the complex healthcare system will lead to a more resource-efficient delivery of high-quality healthcare services. The book synthesizes evidence-based practice and research on the links between healthcare services, employee health and wellbeing, and quality of healthcare from an interdisciplinary perspective. Written by leading experts in this rapidly expanding field of inquiry, this is the first book ever compiled on the subject with such scope and breadth. It discusses how to conduct interventions and research on healthy healthcare with different populations and settings. The chapters critically examine the links between these pillars; and identify research gaps in both methodology and content from the perspectives of psychology, medicine, nursing, economy, law, technology, management and more. This innovative book is of interest to researchers and students of health sciences, public health, health economics and allied disciplines, as well as to stakeholders in the healthcare industry. Care Management In Social And Primary Health Care

eBook Download

BOOK EXCERPT:

This title was first published in 2003: This book provides an evaluation of the Gateshead Community Care Scheme which was devised as an alternative to residential and hospital care for frail elderly people. An important feature of the scheme was the decentralization of control of resources to individual social workers acting as care managers, with defined caseloads and expenditure limits to ensure accountability. The initial social social care scheme was subsequently extended to provide both health and social care to clients from a large general practice based in a health centre. The social care team was enlarged to include a nurse care manager and part-time doctor and physiotherapist. The study examines the operation of care management in both settings, the use of devolved budgets and services developed, the outcomes for clients and carers and the costs of care. Admissions to residential care were reduced and the elderly people who received the scheme’s support experienced a better quality of care and greater well-being when compared with elderly people receiving the usual range of services. This was achieved at no greater cost. The characteristics of those for whom the scheme was most appropriate are described. In addition, the pattern of development of the scheme as it was incorporated into the mainstream of the Social Services and after the implementation of the NHS and Community Care Act are examined. Final, the implications for the development of care management are considered.
