The differentiation of the temporal lobe into a lateral neocortical and a medial allocortical region is supported by developmental, anatomical and clinical evidence. Although this view of a dual temporal lobe is generally accepted by neurosurgeons dealing with functional surgery, it still receives little attention by those approaching structural abnormalities located or extending into the medio-basal region. Consequently, the characterization of the temporo-medial area as a distinct surgical region is still lacking. The major object of this study is to analyse the medial part of the temporal lobe as a distinct surgical region and to integrate the microsurgical and physiological aspects into a concept applicable to the several types of temporo-medial lesion. The study includes five sections. The first section is devoted to the morphological aspects; the second and the third sections present a simplified clinical approach to temporo-medial lesions and analyse the ancillary investigations which are indispensable for characterizing their structural and functional features. The fourth section deals with the surgical aspects of temporo-medial lesions, and especially with the selection of the optimal approach in regard to the location of a given process, and to the extent of the functional changes. The last section is devoted to commentaries concerning the neuropathological aspects and the outcome of surgery in the temporo-medial region.

Preoperative Magnetic Resonance Imaging allows now a better analysis of the relationship between intracranial tumors and the surrounding brain tissue. Even very small deepseated tumors can be identified during the operation by intraoperative ultrasound. These technological advances have had a great influence on indications, planning strategies and microsurgical techniques. The author shows the functional anatomy of the brain with the eye and the mind of a microsurgeon and demonstrates well known topographical relationships under the aspect of an utmost gentle operative approach.

This book provides a comprehensive and practical guide for the safe and efficient management of patients with intrinsic brain tumors and medically intractable epilepsy. It presents in an easily understandable way the preoperative evaluation of these patients, starting from the clinical interpretation of conventional anatomical MR imaging and analyses the clinical significance of newer MR based imaging techniques such as diffusion and perfusion imaging. It demonstrates with clarity the role of MR spectroscopy and fractional anisotropy and diffusion tensor imaging in the preoperative assessment of these patients and how this data can be incorporated into the surgical planning. This book is aimed at neurosurgeons, neuroradiologists, neurologists, and epileptologists, and may also be of interest to neuropsychologists, neurophysiologists, radiation oncologists, and medical physicists.

Modern diagnostic imaging and operative approaches have witnessed significant improvements in our times. Computerassisted methods are in use in all microsurgical fields. Neuronavigation, novel stereotactic methods, endoscopic procedures, magnetic resonance imaging, ultrasound and the progress in pre- and intraoperative epilepsy diagnostics must be mentioned in particular in this connection. However, the insights of neuroanatomy and neurophysiology have not become obsolete thereby, on the contrary: such knowledge is imperative and a prerequisite for all neurosurgeons, nowadays more than ever before. Otherwise, excellent modern approaches are liable to fall into discredit if microanatomical aspects are neglected. The goal of this book is two-fold: first, to guide the resident towards a fruitful application of anatomical basics in visualizing and operative techniques. Second, to draw attention to as many anatomical norm variants as possible to forestall complications during surgery. Standard methods, such as the pterional approach, often confront the surgeon with a range of anatomical variants.

As an addition to the European postgraduate training system for young neurosurgeons, we began to publish in 1974 this series of Advances and Technical Standards in Neurosurgery which was later sponsored by the European Association of Neurosurgical Societies. This series was first discussed in 1972 at a combined meeting of the Italian and German Neurosurgical Societies in Taormina, the founding fathers of the series being Jean Brihaye, Bernard Pertuiset, Fritz Loew and Hugo Krayenbuhl. Thus were established the principles of European co operation which have been born from the European spirit, flourished in the European Association, and have been associated throughout with this senes. The fact that the English language is now the international medium for communication at European scientificconferences is a great asset in terms of mutual understanding. Therefore we have decided to publish all contri butions in English, regardless of the native language of the authors. All contributions are submitted to the entire editorial board before publication of any volume for scrutiny and suggestions for revision. Our series is not intended to compete with the publications of original scientificpapers in other neurosurgical journals. Our intention is, rather, to present fields of neurosurgery and related areas in which important recent advances have been made. The contributions are written by specialists in the given fieldsand constitute the first part of each volume.

Endoscopic neurosurgical interventions gain in importance. This book gives adetailed description of the recent indi- cations of endoscopic procedures inmodern neurosurgery. They include endoscopic stereotaxy, endoscopic evacuations of intracerebral hematomas, cysts and abscesses as well as endoscopic interventions on brain tumors. An extended over- view about the usable endoscopes and the operative equipment is pointed out.
