Prognosis In Advanced Cancer

eBook Download

BOOK EXCERPT:

Predicting survival and other outcomes is increasingly being recognized as an important skill for palliative care doctors and nurses, oncologists, and other healthcare professionals who treat patients with advanced cancer. Accurate prognosis is essential if we are to offer quality of care and 'a good death', as well as to aid decision-making. There is much prognostic information available that is scattered throughout the palliative care and oncological literature but this is the first time it has been gathered systematically in one place. Glare and Christakis, leaders in the field of prognosis, bring together a team of international contributors from across the fields of palliative care and oncology. This comprehensive but practical guide begins with the principles of prognostication, including formulating the prediction and then communicating it. Topics such as statistical issues, evidence-based medicine, and the ethics of prognostication are also covered. The second section addresses prognostication in 15 specific cancer sites once they have reached the advanced stage, following a standard template for consistency and easy access to the key information. The third section deals with prognostication in patients with a variety of common clinical conditions at the end of life, such as bowel obstruction, hypercalcaemia, and brain metastases. In addition, survival curves are provided within each chapter, palliative care conditions are examined for the first time, and a summary table of long and short term prognosis ensures this book remains practical.

Palliative Treatment For Advanced Cancer Patients

eBook Download

BOOK EXCERPT:

This book presents an important reflection on the concept and limits of the Fundamental Right to Health as opposed to a supposed “Right to Hope” in the context of the treatment of patients with advanced cancer. The central idea of the work is the question of whether and to what extent patients with advanced cancer have the right to legally demand a palliative treatment whose efficacy has not been proven from the point of view of the desired objectives. The book demonstrates how hope cannot be subject to legal protection and, also, that, even if theoretical-legal reasons were not sufficient for the absence of an abstract right to hope, ethical reasons would be. The work concludes that the best palliative care, rather than palliative treatment, guarantees the best right to health for advanced cancer patients, especially in terminal cases. In addition to this theoretical discussion, the book also presents the results of a qualitative research the author conducted with 48 advanced cancer patients in Brazil and Germany to investigate their expectations towards chemotherapy. This study has confirmed that many patients decide to undergo often toxic and exhausting treatments, unrealistically believing that their cancer is curable or that, as long as they continue with a course of chemotherapy, cancer may be beaten. Palliative Treatment for Advanced Cancer Patients: Can Hope Be a Right? will be of interest to health professionals and social workers working with advanced cancer patients, as well as to researchers in the fields of public health, bioethics, medical ethics and health law, especially those interested in the growing interdisciplinary field of end-of-life decision-making.

Tasman S Psychiatry

eBook Download

BOOK EXCERPT:

Authored by over 500 internationally acclaimed expert editors and chapter authors from around the world. Completely updated and expanded with almost 40 new chapters. Significantly increased attention to the role of culture in all aspects of evaluation and care. New sections on Digital Mental Health Services and Technologies, Treatment Issues in Specific Populations and Settings, and on Prevention, Systems of Care, and Psychosocial Aspects of Treatment address key advances. This edition is the first comprehensive reference work to cover the entire field of psychiatry in an updateable format, ensuring access to state of the art information. Earlier editions were called “the best current textbook of psychiatry” by the New England Journal of Medicine, and “the gold standard” by the American Journal of Psychiatry. Tasman’s Psychiatry, 5th Edition, builds on the initial vision in prior editions of approaching psychiatric evaluation and care from an integrative bio-psycho-social-cultural perspective. It is designed to be an essential and accessible reference for readers at any level of experience. This editorial approach encompasses the importance of the first encounter between patient and clinician, followed by the complex task of beginning to develop a therapeutic relationship and to develop and implement a treatment plan in collaboration with the patient. The importance of increasing attention to the role of culture and social determinants of mental health is reflected both in specific chapters and in components of many chapters throughout the book, especially in those pertaining to clinical evaluation, the therapeutic alliance, and treatment. The global scope of this edition is reflected throughout the book, including the section on psychiatric disorders where evaluation using both ICD 11 and DSM 5-TR is discussed. Most chapters are authored by experts from at least two different countries or continents, adding a critically important dimension which often is missing in major psychiatric textbooks. Tasman’s Psychiatry, 5th Edition, is an essential reference for all medical professionals and students who need a trusted reference or learning tool for psychiatry, psychology, clinical research, social work, counseling, therapy, and all others.
