This book analyses the complexity of South and Southeast Asia in international health, taking into account the impact of the geopolitics of the Cold War on the development of public health and development in the regions. In light of the recent health pandemic, which has mobilized experts and governments and led to a securitized approach to global health, this book offers a regional approach to global health histories. The chapters provide case studies ranging from the Cold War to the present time and covering countries from across South and Southeast Asia. Contributors analyse issues related to disease control, an adjunct to wider Cold War geopolitics. They also examine the responses of regional organizations, particularly the ASEAN (Association of Southeast Asian Nations) and SAARC (South Asian Association for Regional Cooperation), towards COVID-19. Collectively, the book illustrates how narrowly-conceived global health programs implemented by aid agencies failed to account for the local, national or regional contexts. In 1949, the newly-independent Indonesia inherited a health system that was devastated by three-and-a-half years of Japanese occupation and four years of revolutionary struggle against the Dutch. Additionally, the country had to cope with the resurgence of epidemic and endemic diseases. The Ministry of Health had initiated a number of symbolic public health initiatives – both during the Indonesian Revolution (1945 to 1949) and the early 1950s – resulting in a noticeable decline of mortality. These initiatives fuelled the newly-independent nation’s confidence because they demonstrated to the international community that Indonesia was capable of standing on its own feet. Unfortunately, by the mid-1950s, Indonesia’s public health program faltered due to a constellation of factors attributed to the political tensions between Java and the Outer Islands, administrative problems, corruption, and rampant inflation. The optimism that characterised the early years of independence gave way to despair. The Soekarno era could, therefore, be interpreted as the era of bold plans but unfulfilled aspirations in Indonesian public health. Based on extensive archival research and a close reading of Indonesian primary sources, this book provides a nuanced account of the inner tensions in Indonesian public health during the twentieth century – between a narrow biomedical approach that emphasised disease eradication, and a holistic approach that linked public health to practical concerns of nation-building.

It outlines the phenomenal improvements in public health, for example the increase in life expectancy from 38 in 1949 to 73 in 2010; relates developments in public health to prevailing political ideologies; and discusses how the drivers of health improvements were, unlike in the West, modern medical professionals and intellectuals who understood that, whatever the prevailing ideology, China needs to be a strong country. NGOs and Civil Society in Thailand critically examines the relationships of civil society to nongovernmental organisations in Thailand, and examines the ‘NGOisation’ of civil society, how NGOs are funded and governed, and in what way the NGOs has been shaped to work with the funder. NGOisation is a phenomenon by which the funded organisations are impelled to transform suit their funder as reliable partners. Focusing on Thailand, an Asian country where NGOs have been heavily relied on the public sector for funding, the book analyses the relations between NGOs and their significant funder, Thailand Health Promotion Foundation (THPF), one of the biggest and most influential players in the NGO sector. As the NGO funded organisations are impelled to transform and adapt to become more professionalised, institutionalised, bureaucratised, and depoliticised to suit their funder as reliable partners, their characteristics and relations with the state are complex and interactive. Engaging with key stakeholders in the field of NGO and public governance in Thailand, the book demonstrates how THPF changed the NGO landscape, integrating them and innovatively coordinating non-state initiatives into public governance system. A novel contribution to the study of NGOs and the state, the book also addresses NGO transformation, politics, and governance. This report: (1) identifies the main policies, objectives, and thrusts in the health manpower development (HMD) program of the World Health Organization (WHO); (2) identifies factors influencing or determining these policies, and examines how they have shaped the HMD program; (3) investigates how policy changes and achievements in health manpower development in the WHO Member States could be correlated with HMD policies and programs in WHO; and (4) draws conclusions for the formulation of policies and programs for WHO for the near and long-term future. Part One of the report starts with a review of the background of organized international health activities and the birth of WHO. The structure and functions of WHO are described along with the administrative evolution of activities related to the education of health personnel and the development of health manpower. Part Two discusses the development of HMD activities under the state priorities of WHO: (1) greater supplies of conventional health personnel; (2) improved personnel standards and academic excellence; (3) international standards and migratory freedom; (4) health personnel to serve all people; (5) efficient training and performance of health personnel; (6) preparation for future requirements; (7) training of appropriate personnel for local health needs; and (8) health service needs determining manpower developments. Part Three examines all HMD activities as observed in six countries (Ethiopia, Indonesia, Malaysia, Gabon, Costa Rica, Barbados). A description of overall world trends in HMD is presented with an analysis of the extent of the relationship between health manpower resources and the health status of the national population. The estimated impact of WHO on national health manpower developments is assessed. Part Four summarizes the findings reported in the previous chapters. (JD)
