Fungal diseases have been with us from antiquity; interest in the chemo therapy of fungal disease has exploded in the past decade. To plan and pro duce a book on the topic of antifungal chemotherapy has come as a personal challenge - and something of an eye-opener - towards the end of my re search career. A landmark publication which still merits reading is Antifungal Chemotherapy (John Wiley & Sons, Chichester, UK), edited by David Speller, which appeared in 1980. However, the fact that ketoconazole, the first of the modern, orally active, wide-spectrum antifungals, attracted no more than two sentences in it indicates just how far we have come in the 1980s. A steady stream of original papers and a number of conference proceedings have chronicled this progress in drug research; outstanding among the latter are the proceedings of an international telesymposium, entitled Recent Trends in the Discovery, Development and Evaluation of Antifungal Agents, edited by Robert Fromtling (J.R. Prous, Barcelona, 1987) and volume 544 of the Annals of the New York Academy of Sciences, entitled Antifungal Drugs, edited by Vassil St. Georgiev, and containing papers and posters presented at a most enjoyable 3-day conference held at Garden City, New York, in the autumn of 1987.

Emerging Targets in Antibacterial and Antifungal Chemotherapy offers constructive ideas to researchers that could lead to the discovery of entirely new classes of drugs. The authors emphasize new topics rather than review work on known antibacterials and antifungals, and identify new targets--either the rate-limiting component of a biochemical pathway or a component of the pathway that is susceptible to a ``screening'' or ``rational drug design'' approach. Each chapter reviews the biochemical pathway and its place in the cellular scheme in order to place the target in perspective. The World Health Organization estimates that at least five million people worldwide are infected with human immunodeficiency virus (HIV) Of these about 100,000 are in Asia and Oceania, 500,000 in Europe, 2 million in the Americas and 2.5 million in Africa (Mann, 1989). The acquired immunodeficiency syndrome is characterized by a derangement in cell-mediated immunity leading to opportunistic infections with for example Mycobacterium spp., Candida spp., Cryptococcus neoformans, Pneumocystis carinii, Toxoplasma gondii and Cryptosporidium. The third symposium on "Topics in Mycology" brought together 265 experts from 32 countries to discuss the epidemiology, immmunological and pathogenetic aspects of AIDS and its opportunistic infections in general and fungal infections in particular. Pneumocystis carinii pneumonia is by far the commonest opportunistic infection in AIDS patients. The nature and classification of P. carinii is still controversial. In search for its true taxonomic affinities an introductory paper formulates a number of key questions. Candidosis is another frequent opportunistic infection. A number of papers discuss the possibility that selective pressures may operate on Candida albicans within the AIDS population and influence its nature: this might have an impact on prophylaxis and curative and/or suppressive therapy.

Despite the major developments in the therapeutic armamentarium for the treatment of infection, the morbidity and mortality of this complication remains very high in patients with compromised defences. Cancer and its treatment represents a major predisposing condition to a variety of infections. These adverse events are still with us, in spite of much progress in the therapy of infectious disease, since cancer therapy is becoming more aggressive, yet further lowering the host's capacity to cope with infections. Moreover, the pathogens adapt effectively to drugs, and at a pace that might outrun industry's capability to produce new agents. Finally, new pathogens are appearing as a consequence of both selection and severe immunosuppression. Infection is so common among cancer patients that its diagnosis and management represent a daily challenge to all oncologists, who must continually strive to keep abreast of developments in the area. The present comprehensive review of the most crucial and challenging aspects of the infectious complications in cancer patients will help them to do just that.

Biomedical scientists widely acknowledge that individuals' immune respon siveness is important in resistance to infections by microorganisms, including fungi.
