Glioblastoma is an aggressive incurable primary tumor of the central nervous system. Median overall survival is in the range of 1.5 years even in selected clinical trials populations. Many features contribute to this therapeutic challenge including high intratumoral and intertumoral heterogeneity, resistance to therapy, migration and invasion, immunosuppression. With the access of novel highthroughput technologies, significant progress has been made to understand molecular and immunological signatures underlying the pathology of glioblastoma. Clinical trial designs have shifted from investigating broad “one-for-all” treatment approaches to precision oncology designs. The collection of contributions in this book aim at providing researchers and clinicians an update on different aspects of glioblastoma, i.e. progress in basic, preclinical and clinical research.

The concept of cancer stem cells has great clinical implications. This is due to the fact that small subpopulations of these cells have been identified in a variety of neoplastic conditions ranging from solid tumors to liquid malignancies. Although there are some huge gaps in our current understanding of the role played by cancer stem cells in cancer biology, a growing body of evidence provides strong support for the principal functions of these cells in tumorigenesis. This book comprehensively reviews the role of cancer stem cells (CSCs) in cancer initiation, progression, and resistance to anticancer therapies. The initial chapters examine the methods and procedure of the detection, isolation, and characterization of CSCs. This book represents an up-dated summary of the state of the art of the characterization of cancer stem cell/ cancer initiating cell (CSC/CIC) properties. An overview of the definition and biological properties of CSCs/CICs as well as the role of these cells in determining the resistance to standard and immune-based therapies is provided. It also discusses limitations in the achievement of a definitive biological characterization of CSCs/CICs due to their high extent of plasticity and heterogeneity that is also mutually driven by the interaction of these cells with the tumor microenvironment. The limitations in targeting CSCs/CICs with immunotherapy are also explained together with explorative combination approaches that could increase the susceptibility of these cells to the recognition by immune cells.
