This compact book provides step-by-step guidance in the approaches and techniques developed and currently used at the authors’ center in Düsseldorf for the treatment of a variety of aneurysms. All of the described procedures are minimally invasive and of proven efficacy. In order to assist the reader in fully comprehending all aspects of the techniques, they are illustrated through clear graphics instead of complex photos and radiologic imaging. While the rapid development of endovascular treatment means that it is no longer necessary to employ microsurgery for difficult and hidden aneurysms, proficiency in microsurgery remains essential. For example, full mastery of microsurgical techniques is vital in order to treat middle cerebral and peripheral aneurysms efficiently and with minimal trauma. Minimal invasiveness and optimal cosmetic results have become critically important in enabling the microsurgical method to compete with the endovascular method. In documenting “tried and tested” procedures, this book will be invaluable for both established neurosurgeons and residents in neurosurgery.

This book explains, using the example of endovascular intervention, the development of in vitro simulators for biomedical applications based on the scientific context in the robotics area and the explanation of the medical procedure to be simulated. It presents modeling methods for in vitro representation of human tissue and for representing tissue integrity during endovascular surgery simulation. Additionally, applications of this in vitro vasculature modeling technology are presented: flow control for human blood pressure simulation, computer fluid dynamics simulations using vasculature morphology, catheter insertion robot control and vasculature imaging based on magnetic trackers, and tailor-made scaffolds for blood vessel regeneration.

Microsurgery has brought about important changes and progress in the treatment of cere bral aneurysms in recent years. Only 12 years ago McKissock, one of the most experienced surgeons in the treatment of aneurysms pompared the results of operative and nonoperative treatment. He found that mortality and morbidity of cerebral aneurysms was not significantly affected by whether the patients have been operated upon or not. This view is held by many neursurgeons, and was confirmed by results of the Co-operative Study in 1969. In the review on treatment of aneurysms in Progress in Neurological Surgery in 1969, techniques such as encasement with plastic compounds, piloinjection, stereotactic copper or electric thrombosis and metallic thrombosis and profound hypothermia in anaesthesia were listed. In general, these techniques now seem to be of little more than historical interest. Progress in recent yeats has been so enormous that one may perhaps gain the impreSSion that the main problems in the treatment of aneurysms have been solved and that there are no more to be surmounted. This is dangerous and complacent thinking, such as Lord Moynihan indulged in during the thirties. For this reason we thought that the time had come to assemble a group of experts who could discuss the various unsolved and contro versial problems of aneurysm surgery in the friendly and relaxed atmosphere of a "work shop".

This book is a comprehensive volume on microsurgery in China. It covers the history, general concepts, equipment and techniques of microsurgery. Unlike other books on the same subject which concentrate mainly on plastic surgery and hand surgery, this book introduces the applications of microsurgical techniques in neurosurgery, gastro-enterology and orthopaedic surgery. This is a valuable text as the vast experience of Chinese microsurgery is also included.

More than 40 articles provide an extensive coverage of clinical and basic science advances over the last three years of research on subarachnoid hemorrhage-induced brain injuries. Early brain injury, the new frontier of subarachnoid research, which may be a key contributor to the high mortality and morbidity, promotes collaborative efforts from neurosurgery, neurology, neuro-ICU into other interrelated fields and basic neurosciences. For the first time, subarachnoid hermorrhage research is almost equally divided by early brain injury and cerebral vasospasm, mechanistic investigations and therapeutic approaches, demonstrating a translational feature of the future direction.
