Many people suffer from substance (from nicotine or alcohol to heroin, methamphetamine, cocaine, etc) or non-substance (from gaming/gambling to shopping, internet, smartphone, etc) addiction. Both types of addiction are associated with severe health, economic and social consequences and are far too often result in both familial and personal tragedies. Unfortunately, current treatments are not very effective for either substance or non-substance addiction so novel treatments are desperately needed. For example, for nicotine addiction, despite very well-documented health risks of cigarette smoking and its economic and societal costs, globally, more than 30% of males still smoke, and although many smokers desire to quit, only approximately 5-30% will achieve abstinence with 6 months or more of the best available current treatments. And the story for nicotine addiction is far from unique, presenting a huge challenge for the entire addiction field. Therefore, there is a vital need to improve outcomes for the treatment of both substance and non-substance addictions. Recent work exploring neurobiological mechanisms of the treatment response at the molecular, cellular and systems levels with a broad range of technologies, such as Magnetic Resonance Imaging (MRI), genetics and genomics, have opened new avenues for developing novel treatments through the detection of neurobiological biomarkers predicting efficacious outcomes in addicted patients.

In the years following publication of the DSM-5(R), the field of psychiatry has seen vigorous debate between the DSM's more traditional, diagnosis-oriented approach and the NIMH's more biological, dimension-based RDoC (research domain criteria) approach. Charney & Nestler's Neurobiology of Mental Illness is an authoritative foundation for translating information from the laboratory to clinical treatment, and its fifth edition extends beyond this reference function to acknowledge and examine the controversies, different camps, and thoughts on the future of psychiatric diagnosis. In this wider context, this book provides information from numerous levels of analysis, including molecular biology and genetics, cellular physiology, neuroanatomy, neuropharmacology, epidemiology, and behavior. This includes an overview of the unique challenges that remain in diagnosing these disorders given our still limited knowledge of disease etiology and pathophysiology. The section then provides reviews of DSM-5(R), which forms the basis of psychiatric diagnosis in the United States for all clinical work, and of RDoC, which provides an alternative perspective on diagnosis in heavy use in the research community. Also included are chapters on future efforts toward precision and computational psychiatry, which promise to someday align diagnosis with underlying biological abnormalities.

The purpose of this book is to provide a broad scope of substance use disorder research and how these findings can impact treatment outcomes. The research and outcomes described in this book represent important principles related to identifying and understanding factors related to substance use disorders. The first section is dedicated to methodology including population-based surveys, basic neuroanatomy, chemistry, molecular biology, behavioral models and brain imaging. The second section utilizes this methodology in research related to opioids, cocaine, marijuana, alcohol and nicotine. The book is aimed at both professionals (academics, clinicians, practitioners) and students or trainees.

Chronic pain is one of the most prevalent, costly, and disabling health conditions in the United States. Estimates show that more than 11 percent of the American population suffer from chronic pain, yet the federal pain research investment has been minimal. In parallel with a gradual increased recognition of the problems of treating chronic pain, the opioid epidemic has emerged as a growing public health emergency. The intersection of these two crises lies in the fact that an unintended consequence of treating pain has been an increasing number of opioid prescriptions and diversion of drugs for illicit purposes. In May 2017, the National Institutes of Health (NIH), and the National Institute on Drug Abuse announced a publicâ€"private partnership to develop solutions to the opioid crisis and cut in half the time it takes to develop non-addictive analgesics. To advance the planning of NIH's anticipated publicâ€"private partnerships, the National Academies' Forum on Neuroscience and Nervous Systems Disorders hosted a public workshop that brought together a diverse group of stakeholders from academia, federal agencies, advocacy organizations and companies developing therapeutics for pain and opioid use disorders. Participants discussed potential strategies to accelerate development of non-addictive pain medications and treatments for opioid use disorders. This publication summarizes the presentations and discussions from the workshop.
